Symptoms:

One of the lights stays on all the time Shorted triac.  
Typical causes:
An output shorted to the common terminal
Attempt to drive too many bulbs exceeding the output capacity.
Surge caused by failing bulb causing over current condition.
Board is dead. Possible causes:
Blown fuse.
Input resistor or main capacitor failed due to modified voltage intput (i.e. Dimmer or Modified Sine-Wave inverter, etc...)
